Overview

RN Care Manager MLTC (Chinese) - Remote with 1 Field Day a week!!

Responsibilities

Collaborate with members and their families to maintain the most independent living situation possible.
Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.
Authorize covered services and coordinate care across different payers.
Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.

Qualifications

Current NYS RN License and registration
Associate's Degree in Nursing Required. Bachelor of Science in Nursing (BSN) Preferred
Minimum of 3 years of RN experience
Clinical expertise in geriatrics preferred; managed long-term care experience required
Bilingual Chinese, Cantonese or Mandarin required

Compensation

Annual salary: $105,000 - $115,000
Eligible for a $20,000 sign-on bonus ($5,000 payable every six months, less taxes)
